The Care2x System was first developed by Elpidio Latorilla in Germany since 2000. The idea was to develop an open source framework for hospital information systems based on open source code. The objective was to make an Hospital Information System, a Practice management module, a Central Data Server and Health Xchange Protocol for sharing information. The Care2x System was first developed by Elpidio Latorilla in Germany since 2002. The idea was to develop an open source framework for hospital information systems based on open source code. The objective was to make an Hospital Information System, a Practice management module, a Central Data Server and Health Xchange Protocol for sharing information.

The public project started with the first release of the beta version 1.0.0. in the internet on May 2002. After 3 months there was a several number of people involved in the project. In the following years about a 100 developers have added to the project (comming for 20 different countries), making some parts of the code, giving new ideas and traslating the software to many languages. In recent years there are more than 300 developers working on the system. Fisrt release was developed by PHP and MySQL database engine. In followin years, Elpidio and the developers team, have made the API of the software and the support for Postgress SQL database engine too.
