The medication list is a single list of all medications with an active prescription or an
 
affirmation by the patient for an OTC medication (for active meds) and a single
 
list of all historically prescribed medications or affirmed by the patient as
 
having been taken for OTC medications. The way in which the list is displayed
 
will help the practitioner if there have been a number of medications
 
prescribed for the same problem (hypertension, analgesia, asthma, etc). The
 
ability to sort the list by "reason for prescription" will be helpful. As stated
 
above, if a given medication has been prescribed multiple times (by a single
 
clinician or by multiple clinicians), it should be listed once with the
 
ability to drill down to all instances.
