The Travelers' Electronic Health Record Template (TrEHRT, pronounced treat) is a form of personal health record open source software that was developed to aid travelers access necessary medical records during travel. The data is meant to be useful for emergency situations without requiring constant updating by including mostly static medical information. This was developed as a standard for travel personal health records under the International Medical Informatics Association (IMIA).

TrEHRT is portable and can be accessed through android smartphones, the internet or with thumb drives. Login is not required to view basic information such as blood group, allergies and the contact number of the consumer’s physician.


Features of the template

Basic patient data:

  • Full name
  • Country code(s)
  • Blood group and type
  • Allergies
  • Marital status
  • Mother language
  • Insurance
  • Emergency contact name, relationship to individual and contact information
  • Passport/ID number
  • Language
  • Body weight and height
  • Occupation
  • Spouse name
  • Religion
  • Employer name


Present medications:

  • Medication name
  • Prescription period (from-to)
  • Medication (ATC code)
  • Frequency


Medical history:

  • Diagnosis (ICD-10 preferred or ICD-9 moving to SNOMED when and if available)
  • Allergy (free text now; RxNorm when available)
  • Smoking and drinking status
  • Vaccination history


Test report:

  • Surgical history
  • Laboratory report
  • Examination report


Travel history:

  • Country code(s)
  • Date (from-to)
  • Status


Family heredity disease history:

  • Father, mother disease


Physician contact:

  • Source of regular medical care, eg, physician, clinic and contact information


How to use

  1. Open an account at trehrt.com
  2. Enter your demographic and medical information
  3. Install TrEHRT on your android phone via link on trehrt.com or through android market
  4. Open TrERHT on your android phone and enter your login information


Once viewed on your android phone your information will be saved to your device harddrive so that an internet connection is not needed for future viewing.


Security and standards

TrEHRT uses data encryption, HTTPS, and MD5 security protocols to protect user data. Interoperability is achieved through standards, when applicable, as mentioned above in the features sections. Examples include, ICD-10, SNOMED, RxNorm, and ATC.
