Disclosure

Disclosure is revealing the patient data or facts to others. It is the giving out of information, either voluntarily or to be in compliance with legal regulations or workplace rules (1). Dictionary defines disclosure as the act or an instance of disclosing; exposure or revelation. It simply means the loss or release of information about an identifiable individual.
