Remote monitoring systems permit electronic monitoring of patients within their homes. Remote monitoring systems are connected to clinical practices via wireless bluetooth technology. Examples of remote monitoring systems include weight scales, home monitoring glucose machines, and BP monitors. These electronic tools can play a critical role in assisting patients to remain in their homes and permit providers to monitor their patients without having to make hoem visits. They are a powerful tool in managing patients, mostly elderly, with multiple co-morbid conditions. For example, patients with heart failre can be assessed in their home and avoid unnecessary visits and reduce hospitalization rates. Monitoring systems are part of a powerful health IT tools that can be deployed to support chronic disease management.
