What does CDO stand for? Personally I like the last explanation the best. I think the best way to differentiate if using both terms in an article are to define what the author means by each.

I don't remember if CDO was defined at the source, but it is something like Care ___ Organization.
Yes, the topic should be spelled out and I believe it should include electronic health record (EHR) since they are different!
