X-Ray technician in room taking X-Ray before nurse is aware of order
Nurses at a hospital where CPOE was being implemented really loved the speed at which orders were processed through ancillary services. Pharmacy turnaround was fabulous. However, one nurse reported being shocked when she went to a patient's room to administer medications and found an X-ray technician setting up for a chest film. The order had been processed before the nurse even knew it had been placed. This places the coordinator of care dangerously out of the loop (and the X-ray technician definitely should have contacted the nurse before taking the X-ray).
